I got mine without a problem. There should be a yellow “submit” button at the bottom of the form. If you don’t see it then switch to a different browser (Chrome, Safari, Explorer)
I also had that problem. It’s just cut off. If you will hit the tab button on your computer, it will take you down to the submit button.
