In order to get these rebates, you must buy the products that have the special “Try Me Free” label on them.  You’ll need the “Try Me Free” label and an original cash register receipt to get each of the rebates.

There is a limit of one rebate per household for each of these products.  Rhode Island residents can claim 2 rebates per household for each product.

i purchased 2 in one transaction, 1 receipt.  Does anyone know how this will work?  Can I use a copy of the receipt to send to one of them or could I possibly send both forms in one envelope with just the original receipt?

The rebates say that you must use an “original sales receipt” – which means no copies.  I wouldn’t recommend putting them in the same envelope since they are different rebates that are supposed to be marked for specific departments.  

I always try to work my rebates as separate transactions for these very reasons.   Since you’ve already made the purchase, you could consider returning one of the products  (if it’s not open) and purchasing it again…  or take your original receipt to the store and ask them if they can issue you a duplicate receipt (it’s a bit more official than a photo copy).   One final option – you could contact Physicians Formula directly and ask them for advice.

I was able to really cash in on this MIR in April when CVS had tem on ECB too.  I bought the mascara and eye-liner and love them both.  Not to mention I made money on them!
