That was weird–the form came up & when I was going to start filling it out the page changed to sorry.
Took about 4-5 minutes to load but finally did! Just let it load and you will get it! Thank you very much for the great link.

It was very, very slow & I had to reload the page a couple of times but, I finally got it to go through- about halfway through the survey, my computer automatically refreshed the page & lost what I’d already filled out but, I got it finished without any problems- just took lots of patience- thanks for the info!
